After you have made your appointment at Family Medicine Specialists of Texas, you will receive a link to our digital patient packet. If you are a new patient or an existing patient needing to update your information, please fill this out prior to your appointment.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all necessary information to provide you the best possible care. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing our Patient Education section. We look forward to your first visit.
No Show and Missed Appointment Policy

At Family Medicine Specialist we value your time and strive to provide the best care possible. We understand that sometimes unexpected circumstances arise. However, missed appointments disrupt our ability to serve other patients. To ensure the best experience for all, we have implemented the following policy regarding missed appointments.
1. Missed Appointments: A “no-show” is defined as a patient who misses an appointment without notifying the office in advance. We ask that you notify us at least 48 hours in advance if you need to cancel or reschedule your appointment.
2. Missed Appointment Fee: After two (2) missed appointments, a non-refundable, no-show fee of $100 will be charged to your account. You will be required to pay this fee in full prior to scheduling a future appointment. If paid, this can be used toward copay and/or deductible.
*It is the patient’s responsibility to remember their scheduled appointment time. We send appointment
reminders as a courtesy, but failure to receive a reminder does not exempt you from this policy.
